SHAMROCK FARM CENTRE FOR AUTISM
REGISTRATION PROCESS

Dear Potential Member:

ASNL has come a long way in creating The Elaine Dobbin Centre for Autism from its first formal proposal in 1999 to present day with the main building completed and planning initiated for programs and services. The long-term vision of the center is to provide vocational programs, communication and social language skills training, parent support programs, recreational programs, training/education for parents and professionals, and coordination of off site programs.


The capital support for this project has been immense, with incredible time, energy, and financial contribution expended to enable us to attain the goal of $1.5 million. The capital campaign has now closed and we have realized the new challenge in obtaining sustainable operational and program monies. Achieving this goal is something you can both help us with and, in turn, be helped by The Elaine Dobbin Centre for Autism. By registering with the centre, we can build accurate data from the autism community of Newfoundland and Labrador to support our financial requests from government and other funding sources. The benefit to you is that you will have access to programs and services as they develop.


THE REGISTRATION PROCESS


Please complete the Registration Form and supporting documents enclosed in this package in order to register for programs and services at The Elaine Dobbin Centre for Autism. The individual who is registering must have a diagnosis of Autism Spectrum Disorder and must have documentation to support the diagnosis. We can obtain this documentation with your consent. We also request ISSP/GSP documentation in order to have that information on hand as we offer programs and services (please see form Request for Documentation of Diagnosis of ASD/ISSP/GSP).


There is a registration fee charged to cover our administrative costs per individual. ASNL members are charged a reduced rate. Should individuals choose to become ASNL members before registering with The Elaine Dobbin Centre for Autism, please contact the office at 722-2803 or info@autism.nf.net.


Once an individual is registered with The Elaine Dobbin Centre for Autism, a confidential file will be created on-site. Demographic information such as geographic location, diagnosis, sex, age, etc. will be compiled by ASNL for the purposes of funding applications, advocacy, etc. Personal or identifying information will never be released unless requested by the individual or guardian.


It is also our intent that when we obtain adequate programming funds, we will contact individuals for an in-depth interview regarding area of interest and abilities for programs and services through The Elaine Dobbin Centre for Autism.


DEVELOPMENT OF PROGRAMS AND SERVICES


Please be advised that The Elaine Dobbin Centre for Autism is currently offering Programs and Services. Your input into this process is greatly valued. Complete the attached questionnaire, Programs and Services at The Elaine Dobbin Centre for Autism, with your suggestions.

We are in great need of volunteers who are willing to take on coordinating roles through support groups, educational opportunities, and fundraising initiatives. If you or someone you know is interested, please contact us at the office.
